The first step is to complete an information form. This step allows you to receive personalized information and guidance as you seek God’s direction. If you would like to speak to someone, please call our Initial Contact Coordinators at 1-888-422-6461.
- When you log on, you must first register which will ensure the privacy of your information through our secure website.
- Once you have registered, you will receive a user name to access the form. Married couples register only once and use the same assigned user name and password.
- The husband's part of the form must be completed first, but should not be submitted until the wife and children's data has been entered.
- You may save your information, exit the program and return later to complete it as long as you do not click the submit button. Once you submit the information form, you can no longer edit it.
- You should receive acknowledgement of receipt from your personnel consultant within 7 to 10 working days after submitting your form.
