Getting started in the screening process

The first step is to complete an Information Form. This does not obligate you to a definite or immediate commitment but allows you to have personalized information and guidance to work through the process. If you would like to speak to someone, please call our Initial Contact Coordinators at the number listed below.

Detailed instructions:

• When you log on, you must first register which will ensure the privacy of your information through our secure website.
• Once you have registered, you will receive a user name to access the form. Married couples register only once and use the same assigned user name and password.
• The husband’s part of the form must be completed first, but should not be submitted until the wife and children’s data has been entered.
• You may save your information, exit the program and return later to complete it as long as you do not click the submit button. You may do this as often as necessary before you submit it.
• Once you submit the Information Form, you can no longer edit it unless the ISC/Jman team resets your account.
• You should receive acknowledgement of receipt within seven to ten working days after submitting your form.

You will need an e-mail address where you can receive electronic mail personally. If you do not have such an address, you may easily create one at no cost at one of the following sites: www.juno.com, www.hotmail.com, or www.yahoo.com. You can use any computer with internet access to set up an e-mail address. Free internet is available through www.juno.com. An active e-mail address is necessary so that you can receive an IMB computer generated user name and submit many application forms electronically.

After we receive your information form
You should receive confirmation that the information form has been received, and then will be invited to complete Information Form Part Two. After reception of these two preliminary items, a consultant will be assigned to you, determined by your service classification of Journeyman, ISC or Masters.
